Permanent Supportive Housing Case Manager

Position Summary:
The Permanent Supportive Housing Case Manager is responsible for providing services to the persons residing in permanent supportive housing units, located in the Bloom and the community. Permanent Supportive Housing residents may present with serious mental illness, substance use disorders, and/or other co-occurring issue which have impacted their ability to obtain/maintain permanent housing. The Permanent Supportive Housing Case Manager provides individualized services, tailored to each client, and general supports related to budgeting, rent payment, connection to resources, maintaining a household, and other areas which support their basic needs and enable them to remain stably housed. The Permanent Supportive Housing Case Manager is responsible for collaborating with representatives from local government, other non-profit agencies, and the faith community.

Salary: $55K annually plus benefits (medical, dental, vision, life insurance, 403(b), PTO, holidays.)

Founded in 1988, Carpenter’s Shelter is a highly recognized private 501 (c)3 nonprofit organization located in Alexandria, VA. Located near Old Town Alexandria, we operate two shelters, The Hoffman Center and the Alexandria Community Shelter. Carpenter’s Shelter supports people experiencing and at risk of homelessness to achieve sustainable independence through shelter and housing placement, guidance, education and advocacy. Carpenter’s Shelter serves approximately 900 homeless and formerly homeless children, women and men each year.
Responsibilities:

• Provide general support to residents using Housing First approach and a client-centered and trauma-informed lens to help residents achieve successful tenancy in permanent housing
• Conduct comprehensive strengths-based housing barrier assessments to collect financial, employment, housing, educational, mental health, substance abuse, etc. histories to support the development of short and long-term goals to support housing stability
• Completes monthly home visits to review household maintenance and assess unit habitability
• Provide support to ensure residents pay rent and utilities on-time and in full
• Ensure residents are connected to mainstream resources and community benefits they are eligible for
• Liaison between residents and property management regarding maintenance, grievances and tenant needs
• Assist residents in developing and expanding their support systems; provide linkages to services and mainstream resources based on the household’s individualized needs
• Develop and maintain relationships with community resources and partners to efficiently support and advocate for households served
• Maintain accurate and complete file information, case notes and Homeless Management Information System (HMIS) documentation, clearly identifying goals, action steps and outcomes
• Function as a contributing member of a multidisciplinary team with the organization’s employees, volunteers, and community agencies
• Participate in staff meetings and trainings as directed by supervisor, including but not limited to Housing-Focused Case Management, Motivational Interviewing, Trauma-Informed Care, and Harm Reduction
• Be on-call status as directed by supervisor
• Adhere to the Carpenter’s Shelter Code of Ethics and SEED Values
• Perform other duties as assigned

Telework is eligible up to one day per week.

 

Qualifications and Job Requirements:
• Bachelor’s degree or equivalent experience in social work, human services or a related field
• Ability to develop strong working relationships with households receiving services
• Ability to maintain documentation
• Strong time management and organizational skills
• Proficient skill level in Microsoft Suites (Word, Excel, Power Point, Teams, SharePoint)
• Valid driver’s license and ability to pass a background check
• Flexibility to work outside of normal business hours, if needed, to accommodate client and community needs
Preferred:
• 2 years experience working with a diverse population experiencing homelessness
• Lived experience or direct personal connection to homelessness
